Position is at Fort Osage High School and partners with the Process Coordinator at the High School to support the transition goals and IEP needs of students in grades 9th-12th.

Purpose Statement
The job of Transition Coordinator is done for the purpose/s of supporting the instructional program with specific responsibilities for coordinating transition services for students who receive special education services; ensuring compliance with special education transition services; outreach for agency collaboration; and providing this information to others.  
 
This job reports to Director - Student Support Services
 
Essential Functions
  • Assists students, staff, and parents with career exploration.
  • Facilitates the transition process as related to planning and implementation.
  • Acts as a resource to district special education teams in the awareness and implementation of transition needs and requirements.
  • Maintains contacts with the business community to keep informed of career opportunities, job shadowing, and transition fair.
  • Communicates with employers, parents, school counselors, and Vocational Resource Educator on individual pupil progress.
  • Provides encouragement for continuation of formal education and assistance to pupils in seeking employment on full or part-time basis.
  • Supervises pupils in out of school work/training activities during the assigned work day.
  • Arranges in-district work opportunities and supervises pupils in these opportunities. Partners with area school districts to coordinate transition experiences for students and families.
  • Serves as the district liaison with vocational training facilities, pre-employment transition services, vocational rehabilitation, and sheltered workshop.
  • Assists staff in the development of school to work options.
  • Assists staff in the development of instructional activities and IEP requirements as related to student transition.  
  • Assists IEP teams in transition evaluation and attends IEP meetings/evaluation meetings to discuss and provide input on student transitions
  • Acts as the Outside Placement case manager for students
  • Facilitates Special Education Compliance for the grades 9-12th grade students including working with special education staff to ensure compliance with timelines, documentation, data, etc
  • Assists staff in providing student testing needs associated with state testing
 
Other Functions
  • Performs other related duties as assigned for the purpose of ensuring the efficient and effective functions of the classroom.
